Lecture: Bone and Soft Tissue Regeneration in the Esthetic Zone

COURSE DESCRIPTON:

A variety of atrophic conditions in the esthetic zone require bone and/or soft tissue regeneration for optimal outcomes. Their management requires 1) proper diagnosis, 2) risk assessment to determine the likelihood of achieving success, 3) selection of appropriate technique, material, and protocol, 4) careful surgical execution, and 5) follow-up and management of complications. Although many techniques, material, and protocol are available, emphasis will be placed on those, which are: evidence based, have highest potential for achieving desired outcomes, associated with lower patient morbidity and less chance of complication. In particular, the VISTA technique provides many biological and surgical advantages for soft tissue and bone regeneration. They have gained enormous momentum recently for their ability to be utilized as diagnostic tools, as well as for a vast array of therapeutic applications. Over 5,000 publications are currently. being published yearly on the topic, and this number is only expected to dramatically increase as novel therapeutic strategies continue to be investigated. This talk focuses on the understanding of exosomes including their cell origin, biogenesis, function, and characterization. Thereafter, an overview of their application in regenerative dentistry and medicine is presented including their use as an adjunct to PRF therapy.

Lecture: Graftless Implantology as a Forward-Looking Clinical Concept for Highly Atrophic Arches

“The edentulous patient is an amputee, an oral invalid, to whom we should pay total respect and rehabilitation ambitions” (P-I Branemark, 2005). The atrophic and edentulous jaw can pose a number of challenges for the implant clinician. In simple terms, the amount of bone that remains is insufficient for the conventional placement of a dental implant. A variety of treatment strategies can be employed to enable implants to be placed despite the paucity of bone stock in either the mandible or the maxilla. Conceptually, these strategies follow one of two pathways: either augmentation of the bone, or the novel utilization of the remaining bone. However, successes do not always occur. Immediate loading cannot be performed in all grafted bone situations. More recently, non-grafting solutions that maximally utilize the residual highly atrophic alveolar or extra-alveolar bone for implant therapy have been reported to have success. The emergence of diagnostic imaging and 3D printing technology alveolar bone for implant therapy have been reported to have success. The emergence of diagnostic imaging and 3D printing technology has allowed clinicians to provide individualized patient specific implants that can adapt precisely to the patient’s remaining alveolar bone. Furthermore, short, small diameter, tilted, as well as paranasal, pterygoid, and zygomatic implants that utilize the patient’s extraoral facial bone outside the alveolar process can provide predictable and optimal results with no or minimal bone grafting with less treatment time. This presentation considers and discusses the rationale and range for contemporary graft less solutions in implant therapy, as well as the use of various graft less protocols as alternatives to grafting and conventional dental implant therapy.

Achieving Predictable Horizontal GBR Outcomes with Amnion-Chorion Barrier Membranes

In the past decade, there has been much interest in the use of dehydrated human de-epithelialized amnion-chorion membranes (ddACM) derived from placental tissue, for use in various periodontal and oral surgical procedures given their favorable and unique biological and physical properties. This evidence-based presentation will discuss the use and advantages of ddACM as a biologically active barrier membrane for achieving predictable bone regeneration with GBR procedures.
